A keloid scar is a firm, smooth, hard growth that occurs as a result of excessive scar formation and often occurs after skin injury and rarely can also occur spontaneously without any significant preceding skin injury. They may develop on any part of the body and extend beyond the original wound margin, although they are most common on the upper chest, shoulders, ears, and neck. It is a result of an overgrowth of granulation tissue (collagen type 3) at the site of a healed skin injury which is then slowly replaced by collagen type 1. Keloids are firm, rubbery lesions or shiny, fibrous nodules, and can vary from pink to the color of the person's skin or red to dark brown in color. A keloid scar is benign and not contagious, but sometimes accompanied by severe itchiness, pain, and changes in texture. In severe cases, it can affect movement of skin. In the United States keloid scars are seen 15 times more frequently in people of sub-Saharan African descent than in people of European descent. Worldwide, men and women of African, Asian, or Hispanic descent can develop these raised scars. Some people, however, have a higher tendency to develop a keloid when they scar: such as those with a family history of keloids and people between the ages of 10 and 30 years. Keloids should not be confused with hypertrophic scars, which are raised scars that do not grow beyond the boundaries of the original wound.

My scars look worse than other people’s Based on collagen amount in scars, one can have a normotrophic, atrophic or hypertrophic scar. Keloids or hypertrophic scars are formed because of any cut or any injury to the skin. It could be just a simple inflammation even post pimples, post acne also can get keloids, so there is disruption on the skin surface and in the process of healing, the skin from either side comes and joins to form a normal skin. But in people with tendency to form keloids the skin from either side which comes is going to be exaggerated meaning excess skin production which forms a lump which is called keloid. Better not to exercise them surgically because in that surgical excursion if made a cut again it is going to form a keloid. Best is to give a small injection into the keloid which flattens it. It is a hereditary factor and can run in the families and the person will always be prone to this keloids. To avoid them best way is to stay away from the injuries.

Keloid is basically abnormal scar formation. Some people skin is prone to it and still in the process of understanding how Keloid is formed. So the first treatment for would be a non surgical approach , that would include using ointments massages compressions garments, compression tools it reduce the Keloid scars. A lot of people do respond to it. If the non surgical does not work, then we dominantly invasive procedure which will be a steroid injection in the area, which is extremely slow process because you have to take steroid injection for 4 to 6 months and this will help to reduce the size of Keloid scars and some people it will help to reduce the symptoms of Keloid scars. However this may fail. In some people it might not respond or it might partially responds. In such cases we might need surgery might be of two types, one is to reduce the size of the Keloid, leaving behind small amounts of Keloid and this is treated with injection of steroid after the surgery is done that is known as intralesional excision. In some people we may have to remove the entire scar and may have to resurface the area with skin graft. In a person who has keoidal tendency we have to incirposoaret radiotherapy. The radiotherapy is given immediately after surgery from post op day 1 to day 5. Almost 15 centigrams of radiation is given, this is done to reduce the severity or the formation of Keloid. These are the few indication and the treatment methods of Keloid. We are not sure which one will suit you best or which stage you are meeting the surgeon, you may have met the surgeon after trying the non surgical methods and have tried injections or tried surgery. So depending on what stage the surgeon will tell you what is the appropriate treatment, whether to go for medical treatment or for medically invasive injections or to go for surgery or skin grafting and surgery plus radiotherapy, that all can be decided once you meet your surgeon. #Keloid

It had been treated three times earlier in other hospitals in South India - but it had not regressed and had grown back. The scar was thick and non tender. What problem was the keloid scar causing her? Along with the fact that it was a social embarrassment for her, she also had severe itching in the region. She had tried several injections / surgery / creams earlier in other hospitals but to no avail. What was done here at Richardsons Dental and Craniofacial Hospital? Here, Prof Dr Sunil Richardson examined the scar and adviced the triple therapy - surgery, corticosteroids injection and silicon sheer therapy ( if needed ). There are 2 types of bad scars which are most commonly seen.one is a hypertrophic scar and one is a keloid. A keloid is a scar which has not stopped after reaching a certain point. The body keeps on producing more scar tissue. So the scar keeps on growing and becoming bigger. Some people who will have a keloidal tendency , will see that most of their scars will form keloids. It is most commonly seen in people of the African descent as their skin is more prone to keloids. Second is certain areas are more prone to keloids. The front of the chest, on the sternum, on the shoulder and in the women it is more commonly around the ear lobules. So when you are pricking the ear to put earrings, that scar can produce keloids. What is keloid is it is a scar which is produced by the body which does not stop after reaching a certain level and the body continues to firm the scar and that is what grows up and forms the keloid. Surgically there is no option to remove the keloid, to cure it completely. What we do is an intra keloidal excision. What we do is we remove the center of the keloid, or the bulk of the keloid and leave a thin rim around, because if we remove the whole thing, and we go on the normal skin, then that is going to form a bigger keloid. So we do an intrakeloidal excision and most of the treatments for keloids are conservative. Conservative means that injections that has to be given once a month and there is a combination of steroids and some anticancer drugs. So to prevent the cells from forming more scars, thereby they try to control it. Complete cure may or may not be possible, but a control is definitely possible. As soon as we find that a scar is not settling down with the healing period of 6 months, or it is continuing to grow or it is till red and angry and it continues to itch, these are all the sign s that something has to be done to prevent it from becoming a keloid from it becoming more. So that is when you consult a plastic surgeon or a dermatologist and you take steroid injections so that the keloid is prevented from progressing further and stop it just at step 1.
